A glimpse into our ThriveWell Champion responsibilities:

As a Home Health Aide, your primary responsibility is to incorporate the 5 Drivers of Health, Wellness andLongevity into your client’s compassionate and comprehensive care so clients can thrive in their homes.  You will work closely with clients and their families to develop individualized care plans that address their unique needs and preferences, fostering a supportive and nurturing environment.

These comprehensive responsibilities aim to ensure clients receive transformative, care that enables them to thrive in the comfort of their homes:

  • Holistic Assessment: Conduct comprehensive assessments of clients' physical, emotional, social, and spiritual needs, collaborating with interdisciplinary teams to develop holistic care plans that address all aspects of their well-being.
  • Five Drivers of Health and Wellness:
    • Personal: Help clients establish their purpose, priorities, and plan.
    • Community: Engage, encourage, and empower the client’s participation in both the ThriveWell At Home community and their personal communities based on their purpose.
    • Physical: Establish a routine with clients that focuses on sleep, steps, and strength training.
    • Nutritional: Introduce and encourage clients to participate in the MRS Diet – mostly plant-based, real whole foods, and stop eating before you are full.
    • Spiritual: Encourage clients to embrace meditation, mindfulness, and motivation.
  • Personal Care: Assist clients with activities of daily living, including bathing, grooming, dressing, and toileting, while respecting their dignity and privacy.
  • Mobility Assistance: Help clients with mobility issues to move safely around their homes, using assistive devices as needed, and implementing fall prevention strategies.
  • Medication Management: Support clients in adhering to their medication schedules, including reminders to take them, while ensuring compliance with prescribed dosage and timing.
  • Nutritional Support: Prepare nutritious meals and snacks according to dietary guidelines and individual preferences, promoting health and well-being through proper nutrition.
  • Emotional Support: Provide companionship and emotional support to clients, engaging in meaningful conversations, listening attentively to their concerns, and offering encouragement and empathy.
  • Social Engagement: Facilitate social activities and outings that promote socialization and community integration, recognizing the importance of maintaining connections and relationships for overall well-being.
  • Health Monitoring: Collect and record clients' vital signs, symptoms, and overall health status.
  • Environmental Safety: Ensure a safe and comfortable environment for clients, identifying and addressing potential hazards or obstacles to their well-being.
  • Documentation: Maintain accurate and timely documentation of client care activities, observations, and progress notes, adhering to regulatory requirements and agency policies.

Let’s Explore your Qualifications and Credentials:

  • High school diploma or equivalent; certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) preferred
  • Previous experience in home care, nursing, or caregiving is preferred, but not required.
  • Strong interpersonal and communication skills, with the ability to establish rapport and build trusting relationships with clients and their families.
  • Empathy, compassion, and a genuine desire to help clients achieve health, wellness, and longevity.
  • Knowledge of basic medical terminology, safety practices, and infection control procedures.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Must pass ACHA background checks and health screenings as required by regulatory agencies.
