As a ThriveWell At Home Care Concierge specializing in holistic home care, you will play a pivotal role in delivering comprehensive, patient-centered care within the comfort of clients' homes.   • Minimum of two years of clinical experience in home care, hospice, or case management, with a strong understanding of holistic care principles.
  • Strong assessment, critical thinking, and problem-solving skills, with the ability to develop innovative care solutions tailored to individual patient needs.
  • Excellent communication and interpersonal skills, with the ability to establish rapport and build therapeutic relationships with patients and families.
  • Proficiency in electronic health record (EHR) systems and Microsoft Office applications.
